I am curious, if you make someone a friend do they have priority over other downloaders? or is this just so that you can allow them to see your files?

In previous generations of eMule's mods, it was common to promote friends by giving them a score multiplier so they would advance in queues faster but nowadays its removed as an unfair option. Some shadier modders might still use that but afaik all legit mods have removed that.

One remaining way to use it is like you said, assign friends and only allow them to see your shared files.

Then of course we have friend slot that gives users instant access to your file but that is counted outside the session ul:dl-ratio..ie if you give someone friend slot your other dl's might get slower for a while..its a tradeoff if you're a releaser as you can give first ppl friend slot so your files spread faster.
